- A copy of your Insurance Card
- The Treatment Plan/Medical Diagnosis from your child’s physician, psychiatrist, etc.
- Any new or previous Pre-Authorizations from your insurance carrier
- Prior invoices, claims, or EOB’s from previous ABA providers.
Spark Learning currently accepts both private pay and insurance. Insurance payments and coverage are reviewed on an individual basis. You are ultimately responsible for any services that are not covered by your insurance benefits.

Most insurance plans recognize our program as Applied Behavior Analysis Therapy. Before you enroll, we suggest that you check with your insurance provider about benefits, exclusions, and deductibles to determine whether your plan covers our services.
